Steps for Certification Renewal as an On-Site System Professional

Attend Workshops

To renew your certification as a Certified On-Site System Professional in Utah, you must attend workshops presented by the Utah On-Site Wastewater Treatment Training Program of Utah State University.

NOTE: After you attend the complete training session, we will notify the Division of Water Quality (DWQ), Utah Department of Environmental Quality (DEQ), which is the regulatory agency that oversees the Certification of On-Site Professionals Program.

Locate Your Current Certificate Number

A current certificate number is required to obtain renewal. Find your certificate number on the List of Certified Onsite System Professionals. You may also contact the certification program for your certificate number:

Apply to the Division of Water Quality for Certification

Using the renewal form (also provided in the class manual), apply to the Division of Water Quality for renewal of certification. A current certificate number is required to obtain renewals.

  • Required fees for each certificate issued: $25, payable to the Division of Water Quality.
  • Certification fees may be paid by cash, check, money order, or online using the Water Quality Shopping Cart. On-line payments CANNOT be processed without a corresponding application, so the application form must also be submitted and the "Order Number" must be included when using online payment. You can also call the DWQ office to schedule an appointment to make a payment in the office by calling (801) 536-4300. Payments cannot be accepted over the phone.
  • Levels 1, 2, and 3 certifications are each valid for up to 3 years. A single application may be submitted to the Division of Water Quality for all Levels, or they may be done separately. Each certificate issued will require a separate fee. To be certified at Level 2 or Level 3, current certification at the lower level(s) is required.
  • If application for multiple levels is on one form, only one certificate will be issued, and only one $25 fee is required (*Except for government employees who are required to be certified as part of their job description. Contact DWQ certification staff for more information—contact information is given below).
  • Individuals who have Onsite Professional Certification Level 2 and/or Level 3 may request that their name be put on the Maintenance Specialist list for performing the annual inspections for operating permits for Large Underground Wastewater Disposal Systems (LUWDs). To be placed on the list, check the box on the application form next to County.

Certification Renewal

Once your renewal of certification application has been accepted by DWQ, your name will be included in the List of Certified Onsite System Professionals.
