Steps to Become a Certified On-Site System Professional in Utah

Attend and pass workshops

To become a Certified On-Site System Professional in Utah, you must attend workshops presented by the Utah On-Site Wastewater Treatment Training Program through Utah State University and pass the examinations to become a Certified On-Site Professional.

NOTE: We will notify the Division of Water Quality (DEQ), which is the regulatory agency that oversees the Certification of On-Site Professionals Program, with the names of those who pass the examinations.

Apply to the Division of Water Quality for Certification

Using the application form (also provided in the class manual), apply to the Division of Water Quality for certification.

  • Required fees for each certificate issued: $25, payable to the Division of Water Quality.
  • Certification fees may be paid by cash, check, money order, or on-line using the Water Quality Shopping Cart. On-line payments CANNOT be processed without a corresponding application, so the application form must also be submitted and the "Order Number" must be included when using on-line payment. You can also call the DWQ office to schedule an appointment to make a payment in the office by calling (801) 536-4300. Payments are not accepted over the phone.
  • Levels 1, 2, and 3 certifications are each valid for up to 3 years. A single application may be submitted to the Division of Water Quality for all Levels, or they may be done separately. Each certificate issued will require a separate fee. To be certified at Level 2 or Level 3, current certification at the lower level(s) is required.
  • If application for multiple levels is on one form, only one certificate will be issued, and only one $25 fee is required (*Except for government employees who are required to be certified as part of their job description. Contact DWQ certification staff for more information—contact information is given below).
  • Individuals who have Onsite Professional Certification Level 2 and/or Level 3 may request that their name be put on the Maintenance Specialist list for performing the annual inspections for operating permits for Large Underground Wastewater Disposal Systems (LUWDs). To be placed on the list, check the box on the application form next to County.

Complete Citizenship Form

As of July 1, 2009, the Utah Department of Environmental Quality (DEQ) is required to verify the lawful presence in the United States of an individual at least 18 years of age (includes sole proprietor doing business under assumed name) who receives an Onsite Professional certification. This law also applies for renewal certifications. Applicants for these certifications are required to attach a notarized citizenship form to the application:

  • Complete the citizenship form (also included in the class manual), but DO NOT sign until in the presence of a Notary Public.
  • Attach copy of your photo ID (passport or I.D. from Government (U.S., any state within the U.S., or Foreign) such as a driver’s license—Copy must be of an ID and NOT just a photo.
  • Have the document notarized.

Certification

Once you have applied and have been certified, your name will be included in the List of Certified Onsite System Professionals, which can be downloaded from the Utah Division of Water Quality/Onsite Wastewater Program website.
