Please note: The course completion certificate issued by the Utah Onsite Training Program is not certification as an Onsite Professional by the Utah Division of Water Quality. It may be helpful for tracking continuing education for other professional licenses.
Using the application form (also provided in the class manual), apply to the Division of Water Quality for certification.
- Required fees for each certificate issued: $25, payable to the Division of Water Quality.
- Certification fees may be paid by cash, check, money order, or online. Checks should be made payable to the Division of Water Quality. - If application for multiple levels is on one form, only one certificate will be issued, and only one $25 fee is required. Local health officers should contact DWQ for more information on fee payments.
